Recurring Work Processes
1. Issue of Certificate of Employment and Evaluation (in both Chinese and English version.):
Please come to our division and fill in the application form or MCU Home Page → Work (Login) → To apply E- Form. After president's approval, the HR Division will prepare the certificate. After General Secretary affixes the university seal, the applicant will receive the cerificate.
Working Days:3 to 5 Days.
2. Issuing Certificate of Termination:
The Certificate of Termination will be issued as the applicant is processing his/her termination. For anyone who has left his/her position, please submit an application.
Working Days:3 to 5 Days.
1. For new faculty, please prepare 2 passport photos, fill in all the forms and submit to HR Division.
The applicant will be informed when the IC card is ready.
2. If the card is lost, please fill in the application form and pay the fee in Bursar's Office.

- NHI shift-in:Please prepare the copies of the shift-out application form and ID card.
- NHI shift-out:Please come to our office to fill in the application form.
- Newborn babies NHI:Please prepare the newborn baby booklet and the copy of household registration certificate.
- Resumption of NHI:Please prepare the copy of entry-exit certificate and household registration transcript certification.
- NHI of foreign faculty:Please prepare the copy of ARC.
- NHI of foreign faculty's dependents:Please prepare the copy of ARC and provide Chinese names.
1. Change or renew the NHI IC card due to loss, breakage, personal information change, photo change etc.:
Fill in the NHI IC Card Application Form (please attach a two-inch photo, and copies of both sides of your ID card or ARC card to the form). You can go in person or entrust other people to the branch of Bureau of NHI/ contact office with the payment of 200 NTD for the reissuance. Mailing address: (100) Taipei post 30-200
2. Apply for an NHI IC card due to the card malfunction, first-time application, and so on:
After you fill in the NHI IC Card Application Form, send it to our office, mail it to the branch of Bureau of NHI/ contact office or go to the branch of Bureau of NHI/ contact office for the application.
3. Time for applying for a new card:
(1) You can get the NHI card within 30 minutes when you go to the branch of Bureau of NHI for application in person.
(2) You can get the NHI card after 4 working days when you go to the contact office for application.
(3) You can get the NHI card after 6 working days when you go to the post office for application.
(4) You can get the NHI card after 5 working days when you apply for it by registered mail.
Further education of the faculty:(Please check relevant articles)
1. Faculty applying for further education must prepare the letter of agreement from the institute you apply to and make sure your application gets the agreement of relevant department, school, university and president.
2. Our office will provide three copies of the contract to faculty after the president's approval. The faculty fills in the contracts and sends them back to Human Resources Division where the staff will examine the qualification of the guarantor and get them sealed. One contract is for HR; the other two are for the applicant and guarantor.
Further education of the staff:(Please check relevant articles)
1. The staff who apply for further education must fill in the e-application form before the end of March and ensure that their supervisors and the president approve.
2. The staff who apply for further education must prepare an official document and obtain the approval of their supervisors and president before the semester starts. The staff who take advanced courses during office hours must fill in three copies of the contracts provided by the Human Resources Division and send them back to HR, where the staff will examine the qualification of the guarantor and get them sealed. One contract is for HR; and other two are for the applicant and guarantor.
3. The staff who take advanced courses during office hours must hand in their class schedule to the Human Resources Division within two weeks after the semester starts.

1. The faculty who apply for promotion must prepare the faculty qualification evaluation resume (please access the faculty evaluation system of MOE http://www.schprs.edu.tw ), one representative book, and three reference books within five years, three copies of Chinese autobiography, faculty certificate, the copy of the offer of employment within three years, certificate of mutual writing and relevant information.
2. The faculty prepare their writing and relevant documents to apply to their department. After the first assessment, send them to school evaluation committee and then send them to Human Resources office for the external academic evaluation.
3. After the evaluation, send them to the university evaluation committee through the Human Resources office. Then send them to MOE after the president's approval.
Flow chart:
Propose the resignation a month in advance.
→ Obtain the agreement of the unit director and submit for the president's approval.
→ Receive the job-leaving procedure detailed list, handle the transfer of duties, and return the processing list.
→ Submit the job-leaving certificate and the health insurance exit proof, and complete the procedure.
1. Applicants must propose the resignation a month in advance before leaving the job. After the direct subordinate chief reports to the first-level Executive Director, and obtains his/her agreement, the resignation will be delivered to the HR Division. The HR Division will officially inform the related departments. After being approved by the president, the applicant can conduct his/her job-leaving procedure only on the last day of the employment.
2. Please request a job-leaving procedure detailed list and a job-leaving notice from the HR Division, and handle the transfer of duties with each relevant department. After completing the procedure, the HR Division will immediately provide the job-leaving certificate and the health insurance exit application form.
